NEW YORK LIFE (ADDED LIFE INSURANCE BENEFIT)
Please use the following forms to apply for coverage. These forms are not available to complete online. They must be downloaded, printed and returned to SEBA. Once completed and signed, you have five options to return them: 1) upload them to our site on the following page, 2) email them to info@seba.biz, 3) FAX them to (909) 383-6600, or 4) Mail them to SEBA, 735 E. Carnegie Dr., Ste 125, San Bernardino, CA 92408, or deliver them in person to the same address.
EMERGENCY LINE
If you are involved in an emergency situation after business hours, you can contact a SEBA Representative. A qualifying emergency is anything which places a SEBA member at potential immediate legal risk, such as a Lethal Force Encounter (LFE), in custody death (ICD), a traffic collision that results in a death or other similarly serious incidents.
To contact a SEBA Representative after hours just call SEBA’s main phone number at (909) 885-6074 and the answering service will contact the on-call representative. We can and will respond any time of the day or night.
