SEBA RIDESHARE PROGRAM
REIMBURSEMENT FORM

ACTIVE Members in good standing can use any licensed transportation service (Taxi, Uber, etc.) of their choice for a safe ride home or to a place where they are staying. SEBA will reimburse members up to $40 per calendar year.

Please submit the following form along with a receipt for reimbursement. This information will be confidential and will only be used for reimbursements. Must submit form and receipt within 60 days of ride. Receipt must indicate date/county of purchase.

This reimbursement program is to promote safe driving by providing an alternative to operating a vehicle when there is a possibility of impairment. The program is NOT intended to be used as supplemental transportation to fulfill the memberships' errands, appointments, etc.

SEBA is not affiliated with any ride-services and is not liable for the service provided.
